Alzheimer's and Individuals with Special Needs Assistance Initiative
As part of our mission to protect life and to assist the public-at-large in a manner consistent with the rights and dignity of all persons, the Exeter Police Department offers a program for individuals with Alzheimer’s and Special Needs to rapidly respond in the event that an individual wanders off, becomes lost or has a medical emergency.
If you are the caregiver for someone living in Exeter who has been diagnosed with Alzheimer’s, or an elderly person who tends to wander or get lost, or an individual with special needs, please click on the link below and submit it online directly to Detective Bailey Teixeira.
This simple, one page form includes information that will help us identify the person, such as height, weight, hair and eye color, as well as provide names of emergency contact people and medical information such as preferred hospital, primary care physician, allergies and medications. It is necessary to have a photograph of the person.
All of the information you provide will be entered into our computer system along with the photograph. The individual will be provided with a safety ID card if one is requested.
If the individual becomes lost, call the Exeter Police Department at 772-1212 immediately and advise that the individual is a participant in the Program. An Officer will be dispatched to locate the missing person.
